Your First Visit

At your first visit with ARCHER Physical Therapy, you will need to bring the following:

Pre Exam Form

Privacy Practice Notice - HIPAA Form (see below)
Express Registration form (two pages - see below)
Your wallet or checkbook - if you wish to use your insurance, we will assist you in filing on your own (See Billing for more information)
Your driver's license or other form of ID
Wear or bring comfortable, loose-fitting clothing
If you already have one, the prescription from your doctor
Don’t be late as we have a strict late policy
Come about 15 minutes early so we can finish your paperwork

When you arrive, you will complete your registration and be directed to a private treatment room where you will discuss your symptoms and history with the physical therapist. You will receive more information about what treatment will be appropriate for your situation, and the options that are available.
